Steps to Registration

This form will walk you through all three steps to register your child. After completing the contact information form below, click on submit and you will be taken directly to steps #2 and #3.

Following this form you will still need to complete:

    • Step #2 – Download and sign Consent Form and Background Check Form show me
    • Step #3 – Pay Mentorship Donation. show me

  • Other Parent in Different Household

    If shared custody with someone in a different household, please provide contact information for other parent

  • Emergency Contact Information

    Please provide an alternate emergency contact name and phone number if parents cannot be reached.

  • Nanny/Caregiver Information

    Complete this section if a nanny or caregiver will be present when a mentor visit your home.

  • Demographic Information

    Providing this information helps Athletes For Kids receive critical funding. As with all of the information we collect, your demographic information will be kept confidential and never shared.
